Consult Language to Request a Referral for Tai Chi

I would like to request an Official Community Care (OCC) consult for Tai Chi through the Mission Act. 

Tai Chi is now available as an official community care consult within VHA's Computerized Patient Record System (CPRS), where local and online (telehealth) instructors are utilized the same as surgeons and specialists in the community.

The Standard Episode of Care (SEOC) selected is Community Care Complimentary Integrative Health (CIH)-Tai Chi.

I would like to request that Chris Hitchens, a certified Tai Chi for Veterans instructor, NPI# 1114588944 be listed as my preferred provider.

Chris is fully credentialed in the VACCN and OPTUM networks.

Please have the Office of Community Care (OCC) or a Care in the Community (CITC) representative load the CCN consult into HSRM and/or fax it to Chris Hitchens at (412) 981-2392.

If there is trouble getting a referral

"Pursuant to Federal Law 115-182 (The Mission Act) I would like to request Tai Chi through the Community Care Network. Public Law 115-182 Statute 1397 explicitly states that if a veteran faces an excessive driving distance to access care that this is considered to provide said care through the Community Care Network. I feel that I meet the aforementioned criteria. I would also like to keep myself safe from Covid-19 due to the rise in the variant. I feel that using a CCN provider who provides these services via Telehealth is a reasonable solution.
